# AfterCare: Understand Your Hospital Discharge Paperwork

> AfterCare turns hospital discharge paperwork into a clear recovery plan — medications, appointments, and warning signs from your own document.

Welcome to the AfterCare documentation. AfterCare reads the paperwork you leave hospital with and turns it into a structured recovery guide — medications, follow-up appointments, warning signs, and a daily check-in — all traced directly to your uploaded document.

<Note>
  **AfterCare never invents clinical information.** Not a dose, not a date, not a warning sign. Every item in your recovery plan is traced to a specific line of the document you uploaded. When AfterCare is not confident in a piece of information, it tells you so and points you back at the original text.
